Receipt #

6 digit number DBI imposes at permit issuance for computer tracking purposes.

Location

Location of property that has permit assigned to it

Frontage

A structure may be bounded by several streets. Frontage will identify the street where the contractor will occupy the right of way. Maybe different from street name (see item 4).

Linear Footage

A direct line measurement in feet that the contractor has paid to occupy the street. Typically this will coincide with the discussions of the structure at street address and be computed in 20-foot increments.

Start Date

This is the first day that the contractor may occupy the frontage. Typical duration is computed in monthly increments.

End Date

The last date that the contractor may occupy the space based upon permit agreement.

Hours of operations

The hours the street frontage may be occupied. Typically is 7:00 AM – 6:00 PM. See special conditions.*

Special conditions

There may be special conditions with restrictions on the street occupancy. The BSM inspector determines these conditions at a pre-construction site meeting.

Construction Company Name

This is the name of the general contractors or contractor occupying the frontage. Noted on placard.

Contact person

This is the responsible person for the contractor.

Office phone

Contractor’s or responsible person’s office phone number.

Cell Phone

Contractor’s or responsible person’s cell phone number.

*Notes: Information is subject to change each 24 hours. Contractors may notify hours of operation, linear footage, and special conditions at anytime through BSM.

Minimum permit duration is computed on a monthly basis, not to exceed six months for anyone permit. Any permit duration may be extended prior to expiration of current term.
